Here is my perspective on Kegan’s Stage 4 and Stage 5 which renders any idea of “Stage 6” erroneous:
Stage 4 is the fullness of singular coherence.
Stage 5 is the question of the incoherence beyond any possibility of incorporation into a singular coherence. What does it mean to engage with that?
(Note, here, the pre/trans fallacy: Stage 1 is the incoherence prior to any coherence, and that is very different than the incoherence beyond coherence.)
The “answer” to engaging with the incoherence beyond any possibility of incorporation into a singular coherence: a singular coherence can enter into communication with other singular coherences.
Any stage requires a coherent previous stage. We cannot talk of “the incoherent mess of incoherent messes…”. There is no meta-incoherency. Stage 4 is the “final” coherent stage. Stage 5 is as far as we can go, because its previous stage is incoherent. We CANNOT have a Stage 6, as no Stage can arise on the ground of an incoherent previous stage.

My theses:
these various developmental maps don't cover different territory but rather *map the same territory at different levels of detail *and select different features of prominence.
Kegan's map is more Zones than Stages and covers the same territory as the other maps. As Zones, it is low-resolution compared to some maps, and this is a good thing, as it makes it more useful to the non-professional.
Establishing a close analogy:
Consider human energy systems, diagram below.
The 3-Zone system is basically: which system of energy production is dominant. It is simple to use when running a race, even for an amateur.
There are other zone systems that address the same diagram, for example, a 9-zone system, which notes every distinguishable point on the diagram: the endpoints, the peaks, the crossings, and more. This is useful for analysis of an athlete by a professional coach in a laboratory setting (athlete on a treadmill, mask on to measure oxygen usage, taking blood repeatedly to measure biological markers).
3-Zone maps and 9-Zone maps refer to the same set of energy production curves. Each system has complete coverage. In 3-Zone system, there cannot be a Zone 4. In a 9-Zone system, there cannot be a Zone 10.
For most amateurs, a 3 or 5-zone system is best until they have practical reason to outgrow the complexity of it. A 9-zone system is inappropriate for the beginning runner!
Back to developmental maps:
Many developmental systems have complete coverage.
Just as you have used "Stage 4.5" to point to a common location within Kegan's stages, Michael Common's 16 Stages could be mapped to Kegan's as Stage 1a, 1b, 1c through 5a, 5b, 5c.
These more complex maps may be excellent for the professional psychologist or developmental coach! But I suggest that they are overelaborated for use in everyday life by the non-professional compared to Kegan's Stages.
In conclusion:
When we compare maps stages, we need to distinguish claims of having maps that cover more territory vs. maps that cover the same territory but in a more detailed way or with different features. These competing developmental maps are the latter.
The more elaborated developmental maps may be appropriate/useful for professionals but generally have inappropriate complexity for effective use outside that context.
